Job Summary:
We are seeking an experienced Occupational Therapist to join our team at Worcestershire Acute Hospitals NHS Trust. As an Occupational Therapist, you will be responsible for delivering high-quality care to patients on 7 wards, predominantly acute medicine. Your expertise will be essential in ensuring patients receive the best possible care and support for a safe and timely discharge.
Key Responsibilities:
- Deliver occupational therapy services to patients on 7 wards, including assessment, treatment, and discharge planning.
- Work collaboratively with the multidisciplinary team to ensure seamless care delivery.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Conduct manual handling assessments and provide recommendations for equipment and support to facilitate discharge.
- Participate in proactive discharge planning to ensure patients receive the necessary support for a safe and timely discharge.
Requirements:
- Experience of leading a team and supervising others.
- Knowledge and experience of working with a variety of patients in an acute physical health setting.
- Experience working with an MDT, as well as carrying a caseload, writing clinical notes and supervising others.
- Ability to work independently and as part of a team.
