Complex Discharge Facilitator

**As a Complex Discharge Facilitator, you will**: Support the ward staff with complex discharges taking the lead in discharge arrangements for individual patients where appropriate, liaising effectively with all members of the Multi-Disciplinary Teams in Community settings. Act as a resource for patients, relatives and carers requiring information on discharge processes, services available and all other issues related to discharge. Clinical Responsibilities Co-ordinate clinical and support services relevant to discharge planning. Ensure effective two-way communication with members of the inpatient and community MDT so that patients receive timely optimum care.
Work as an integral part of the MDT across the wards and participate in patient assessments where required. Maintain records in accordance with the Trusts policy and that of the relevant professional bodys Standards for Records and Record keeping. Participate in audit programmes and data collection appropriate to the role. Identify the need for and coordinate multi-disciplinary team meetings/case conferences and support ward staff in the decision-making processes with regard to the safe, effective, and timely discharge of individual patients.
Promote and support patient advocacy, choice, self-empowerment and encourage patient independence, ensuring full engagement in decisions regarding their future care needs. Participate in discharge planning meetings concerning the management of patients with complex needs. Work in close collaboration with the allocated care coordinator to enable a timely assessment of the patients individual needs and ensure the appropriate package of care is implemented. Communication Contribute to the building of effective communication systems for community services, social care, other care agencies, the voluntary sector, and the patient the patients family and or the carer.
Attend and present specialist assessments to commissioning panels acting as an advocate for patients. Support MDT to set up healthcare packages for patients and contribute to the decision making with respect to the need for referral to other agencies and disciplines as appropriate. Liaise effectively with statutory and voluntary agencies in relation to the patients discharge arrangements. Establish and maintain communication with individuals and groups about difficult or complex discharges/transfers overcoming any problems in communication.
Ensuring effective communication pathways that promote the appropriate exchange of information regarding a patients plan of care. Involve, support, inform and educate the family/carers where appropriate through the discharge process.
