We are seeking a highly skilled and experienced Social Worker to join our PARTNER Repatriation Service as a Complex Discharge Facilitator. As a key member of our team, you will play a vital role in ensuring smooth and timely transitions for patients with complex mental health needs from out-of-county enhanced rehabilitation units into community-based services.
Key Responsibilities- Coordinate clinical and support services relevant to discharge planning, ensuring effective communication with patients, families, and healthcare professionals.
- Identify the need for and coordinate Multi-Disciplinary Team (MDT) meetings and case conferences to support safe, effective, and timely discharge of patients.
- Carry a caseload and work collaboratively with community services to support timely discharge from locked rehabilitation/specialist placements.
- Participate in all aspects of discharge and aftercare, including cooperation with Section 117.
- Ensure that risk assessments are carried out in conjunction with other agencies, and attend commissioning panels as an advocate for patients.
- Establish and maintain communication with individuals and groups about complex discharges, overcoming any problems in communication.
- Involve, support, inform, and educate family/carers throughout the discharge process.
