**ADMINISTRATION ASSISTANT- Discharge Support Service (DSS)**

**Reporting to**: DSS Service Manager

**Salary**:£6,565.10 for 12 hours (£19,148.22 FTE)

**Hours of work**:12 hours a week - Working days can be discussed but a **minimum of 6 hours must be on a Tuesday.**

**Background**

Age UK Buckinghamshire reaches around 4,000 older people a year with services ranging from support with isolation and reconnection to communities via our Befriending and BOOST teams, to assistance via our hospital Discharge team, Information and Advice and Welfare Benefits advisors.

The Hospital Discharge Support Service works with people who are returning home after a a stay in hospital. The Discharge element of the service takes people home from hospital and ensures they are settled in well at home. The Community Support element provides short term, focused help at home with practical tasks whilst the individual recovers and is able to manage independently.

**Job Purpose**

This is a pivotal role providing administrative support to the Discharge Support team to promote the provision of a quality service to our clients. The role will be responsible for, answering client and professional enquiries, maintaining accurate records using Salesforce and office administration. You will be working closely with the Service Manager alongside members of the team. This is an office-based role, based at the Age UK Buckinghamshire Offices in Aylesbury.

**The role**

You will be responsible for providing excellent administration support to the team to ensure the efficient running of the service. You will be an excellent communicator, able to manage a wide range of enquiries from both clients, carers and professionals. You will be able to manage a busy a varied workload and work well with conflicting demands.

**Key tasks and responsibilities**
- Input and maintain accurate and up to date service user information using ‘Salesforce’ database
- Provide administrative support to the Discharge and Community Support teams
- Make welfare telephone calls to service users as directed
- Signpost to further support available; liaising with Community Connectors, external organisations, partners and voluntary and community sector colleagues
- Ensure that office facilities and stationery are maintained
- Provide support for specific operational tasks as and when requested
