The RAPID programme which stands for ‘Rapid Access to Pulmonary Investigations and Diagnosis’ is a new project launched by the lung cancer team based at Wythenshawe Hospital to dramatically reduce the time taken to complete investigations for patients where there is any suspicion of lung cancer. the team covers lung cancer diagnostics, radiology bookings and the CURE team managing patients with tobacco addiction.

The Surgical Patient Navigator will be responsible for the co-ordination of the patient pathway through the Thoracic surgery service, tracking patient's investigations and ensuing they're booked to surgical appointments within targeted waiting times. They will also contribute to avoiding breaches and working to the National cancer targets, as well as preparing MDT meetings, as this roles requires cross cover with our lung coordinators.

Previous knowledge of working in a relevant hospital based role and/or in a clerical role is essential. An understanding of National NHS waiting time targets would be desirable.

In return we will provide a comprehensive induction programme and you will be appraised on a yearly basis with a view to developing knowledge and skills where appropriate.

Be responsible for the analysis and adjustment of complex patient pathways and able to support and advise both internal and external clinical teams and patients.

Ensure that every patient and health care professional that utilises the MFT surgical service has an excellent experience of it.

Provide both verbal and appropriate written information to patients about their forthcoming appointments and procedures, as well as booking services such as translators and transport.

Assist medical staff to triage patients into the service and identify those patients suitable for prehabilitation and clinical trials.

Track patients throughout their journey, against the relevant patient pathways ensuring that patients are managed well within the relevant national target, liaising with the relevant ward/department. Take appropriate action/escalate any delays with the relevant clinician and directorate management teams to prevent target breaches. Daily use of the live tracking boards.

Use judgement involving facts and situations when escalating issues to lead clinicians and Trust management.

Feedback to relevant departments on investigation results, agreed treatment plans, occasional completion of further investigation forms, e.g. CT scans.
