An exciting opportunity has arisen for a Band 6 Community Care Coordinator to work in our lively and friendly team based in the multi-cultural city of Bradford. We are looking for skilled, enthusiastic, and passionate clinicians to join our fantastic City and North Community Mental Health Team for Older People. As a band 6 clinician you will be expected to act as a role model and resource for our less experienced staff. You will also have opportunity to be involved in service delivery improvements and innovations.

You should be caring, flexible, and creative. Able to thrive under pressure, you will know how to connect with people on all levels. You will be committed to the delivery of high-quality service, working both independently and as part of an established team. You will act as a role model and resource for our less experienced staff and students and will also have opportunities to be involved in service delivery improvements.

As a Community Care Coordinator, you are not only supporting clients through what can often be the most challenging of times, you are also mentoring them to become the best version of themselves. Having an open mind and a flexible approach to care is key. This helps to establish the best possible ways to care for your clients within a recovery-focussed mindset, ensuring that you never lose the motivation that change is achievable for all. All care that you deliver will be in line with current care pathway frameworks and national guidelines.

Working within our multidisciplinary team, you will enjoy being part of an organisation that relies upon the professionalism, insight, expertise, and passion of its staff to inspire individual recovery for the people we work with, whilst also nurturing, developing, and encouraging our staff to achieve their career aspirations.

Successful applicants will be able to demonstrate a high level of commitment, motivation, and reliability.
