Advanced Clinical Practitioner

Are you an experienced Advanced Clinical Practitioner looking to work in an outstanding CQC rated Trust? Dorset Healthcare are expanding the Community Intermediate Care Teams who deliver D2A, Admission avoidance, Virtual Ward and Urgent Community Response, we want you to join our dynamic, positive multi-professional team.
You will be established and confident with respect to complex clinical reasoning. You will have a keen interest for Older Peoples medicine and frailty. You will need to be comfortable with a high degree of clinical risk taking a whilst thinking outside of the box. As the Advanced Clinical Practitioner for our Community Intermediate Care Teams, you will have a passion to help influence and shape the future direction of the role and service. This is an exciting scope of work with a variable and developing pace of change.
In return you will have access to our network of frailty ACP’s who are available to support you. The Trust is also implementing a program of comprehensive changes relating to advance practice including supervision and ongoing development. This ACP post is a full-time role, we would welcome part time applicants with a minimum of 22.5 hours. The role will cover the west of Dorset, this can be discussed further at interview.
As an Advanced Clinical Practitioner within the Community Intermediate Care Teams you will conduct acute assessments to plan and manage acute episodes of care. You will undertake independent, comprehensive assessments which will involve complex clinical decision making to plan, implement and evaluate the care delivery according to changing healthcare needs and varying degrees of clinical risk.
You will be responsible for providing expert clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis, and advanced decision-making skills.
You will lead as a driver for change, to monitor and improve standards within the service through supervision, evidence-based practice, clinical audit, research, and education.
