As an Advanced Practitioner within our Continuing Healthcare Hub (CHC Hub), you’ll play a vital role in delivering our transformation programme for Adults, Housing and Community Services - working together to ensure our services strengths based, co-produced, can manage demand and are financially sustainable.

Within our CHC Hub, you’ll ensure the delivery of the responsibilities of the Local Authority within the Continuing Health Care National Framework.

Training will be provided, and you’ll be fully supported as part of the leadership team to grow and develop within this role.

You’ll benefit from partnership working and be eligible for both local and national training courses related to Continuing Healthcare, Section 117, the Care Act, joint funding, and inter-agency development.

**What you can expect to be doing**:
Our Adults and Housing Directorate delivers adult social care, housing, and a range of community services to people of all ages living in Dorset within the context of the Dorset Council Plan.

The Council Plan is focused on making Dorset ‘a great place to live, work and visit’. This will be delivered through 5 priority outcome areas: Staying Safe and Well, Economic Growth, Unique Environment, Suitable Housing, and Strong and Healthy Communities.

With full support of our Specialist Manager, you’ll lead on specific responsibilities to support the professional development of the officers within the CHC Hub, comprising of qualified Social Workers and Business Support colleagues.

You’ll work closely to support designated staff with implementation of the Care Act, the NHS Continuing Healthcare National Framework and other relevant legislation including changes associated with the social care reforms and CQC quality assurance framework.

You’ll also lead developments in quality of practice, ensuring that practice is compliant with legislation and delivers the Local Authorities responsibilities. Supporting the development of learning and development opportunities of your team, you’ll ensure these are aligned with professional development requirements.

You’ll support the development and implementation of workstreams within the transformation programme, contributing to the design and implementation of new operating models for Adult Social Care. This will also involve contributing to the leadership of the directorate work, working closely with colleagues in other workstreams to ensure a collaborative approach.

**About you**:
To be successful in this role, you’ll be able to evidence your previous management experience, including assignment of work, managing balance of risk and supervising team members.

You’ll be prepared to represent Dorset Council at both operational and strategic levels across NHS England and Integrated Care Boards, in developing policy and improving collaborative working with partners across the local (and national) health and social care system. The role will include responsibility for attendance at Independent Review Panels.

You’ll be comfortable with role modelling the behaviours that are required to make Dorset’s adult social care a modern and progressive service - leading change and valuing the diversity and experience of the adult and their family to improve outcomes.

You’ll be competent with involvement in audit work, quality assurance and budget meetings and will provide regular performance and quality reports as required.

Your relationship building and communication skills will enable you to liaise effectively with local elected members, work collaboratively with partner organisations within Dorset ICB and a wide range of stakeholders, as well as participate in directorate meetings and whole council working groups.

This position has a significant travel requirement. This means that there is a requirement for a vehicle (or transport deemed to be suitable by the county council) to be available on most working days to carry out normal duties.

This role can accommodate someone who is a qualified AMHP (Approved Mental Health Practitioner), however it is not a requirement for the role.

The base location for this role is negotiable. Interviews will be held in County Hall, date TBC.
