An exciting opportunity has arisen for someone who is highly organised with excellent communication skills to join the Risk and Governance team as a Risk and Governance Facilitator.

Risk and Governance is within Quality Governance under the Chief Medical Officer's Office.

The post holder will have a range of generic responsibilities and will specialise in key areas of risk and governance. They will work as an integral part of the relevant directorate/division, supporting staff to identify areas of concern and risks that may impact the delivery of safe care.

The post holder will be required to ensure that working alongside the directorate/division is conducted in line with the requirements of the central risk management and governance function, ensuring essential lines of communication and working closely and coherently with the central risk and governance support team, Risk and Learning Manager and Head of Risk.

The post holder will also be required to develop, maintain and strengthen key working relationships with divisional managers and other senior staff across clinical and non-clinical directorates/divisions, as well as external agencies, and in so doing ensuring the trust is at the leading edge of improving the quality and safety of its patient services with a systematic and disciplined approach to improvement.

The post holder will work directly to the Risk and Learning Manager and will work closely with the governance support teams who have responsibilities for clinical and non-clinical risk management, patient safety, ensuring co-ordination of activities and avoiding duplication of effort.

The post holder will also be required to develop, maintain and strengthen key working relationships with Group Managers and other senior staff across Clinical and Non-Clinical Directorates/Divisions, as well as outside agencies, e.g. Commissioners, Clinical Networks, other Acute Providers, Local Authorities, Independent Sector, and in so doing ensure that the Trust is at the leading edge of improving the quality and safety of its patient services with a systematic and disciplined approach to improvement.

Reports
- Responsible for producing regular reports with the focus upon division, directorate and organisation risk management, risk performance and key learning for local governance meetings and other Trust meetings as appropriate.
- Provide key patient safety information to the divisional management team and specialty leads, supporting risk management. Analyse a range of risk data, identify discrepancies, trends, hot spots and work with the relevant Division Management Leads including the Governance Lead to identify solutions and help ensure these are implemented throughout the division/specialty as required.
- Responsible for producing in-depth monthly integrated governance divisional reports focusing on incidents, serious incidents, risk register entries and highlighting and monitoring any concerns that have been identified including themes and trends.
- Identifying themes and analysing trend reports for incidents (including CCG incidents) and triangulate with other key areas such as claims, complaints, clinical audit, and mortality wherever possible.
- Support with and contribute to the Trust-wide and any division level Learning Lessons newsletters.

Incidents and investigations
- Review incidents daily (Mon-Fri) to monitor the divisional reported incidents, identify risk themes and escalate accordingly.
- Where risk issues and/or incidents do occur, provide support and advice to ensure that relevant incidents are investigated, lessons are learned and remedial actions are taken and sustainable solutions are introduced.
- Support compliance with the requirements for reporting, investigating and management of incidents, including Serious Incidents (SIs) in line with Trust Policy and for ensuring investigations are undertaken within the agreed timescales and that statutory and mandatory reporting requirements are met.
