Cdc and Pacing Pathway Co-ordinator

**Key Responsibilities**: 1. General Duties Prepare the dedicated pacing notes for the weeks clinics and put them away when complete. Answer the devices telephone. Schedule patient clinical queries for a call back during the next available pacing clinic.
For the consultant devices clinic plan forward according to consultant availability and add to Solus. Coordinate the patient and devices notes for this clinic ensuring the reason for referral is present, make new appointments and send to typing pool. Add comments or reports to Solus for consultant clinic and any MDT meeting. Arrange MDT device notes for review and book any appointments or extra tests.
Schedule the biweekly MDT meetings on Solus so no patient are booked at this time. Monitor Pacenet data and ensure is correct for all patients. Add transfers from other hospitals to PAS and Solus and ensure appropriate first follow up is scheduled. Help to make up new patient implantation notes, print ID cards, schedule follow up and add Pacing implants to NICOR.
Request and track hospital notes for the weeks clinics for pacing, PLED echo clinic,ETTs and Tilts. Book and cash up appointments on PAS and ensure that they are accurate according to the Solus record for pacing and PLED echo patients. Book follow up appointments on Solus according to the required follow up period as advised by the physiologist for pacing and PLED echo patients. File all results in the hospital notes and track back out when complete.
2. Performance and Review Assist the technical staff to ensure all investigations are carried out within the target timescales set, alerting them to any waiting target problems in a timely manner. Perform junior administrator performance reviews. 3.
Service Planning Manage the clinic bookings ensuring clinics are utilised efficiently. Organise registration of new patients. Advise patients how to book transport, liaise with ambulance control and patients to ensure they have transport arranged for outpatient appointments if needed. Register patients onto the database, book appointments and send out appointment letters.
Order, trace and collect case-notes as required, liaising with medical records and other departments as required. Ensure notes and urgent reports are delivered in a timely manner, and ensure any departmental photocopying is kept up to date. 4. Financial Management Assist with the ordering of stationery and ensure stationery is checked and stores are kept in an organised manner.
5. Clinical Governance/Quality Be the first line contact for patients with complaints ensuring they are handled with patience and tact in an appropriate manner, and advising them of the correct procedure or referring them to the physiologist in charge as appropriate. Take minutes at team meetings and mortality meetings as required. Liaise with the IT department and external companies to help ensure the IT systems are functioning at all times.
