We have an exciting opportunity for an experienced Clinical Team Lead to join our Early Intervention Community Team within Birmingham Community Health Care NHS Foundation Trust, the largest community trust in the country.

Part of a wider Early Intervention programme, which has so far delivered: 77000 fewer acute bed days used, 19000 fewer non-acute bed days and people spend 11.5 fewer days in the system. Overall having an impact of £25.8million saved for Birmingham to reinvest in Health and Social Care developments.

You will be joining a community team in the midst of one of the largest Health and Social Care transformations in the country, with exciting changes and developments, preventing avoidable hospital admissions, and facilitating early hospital discharge.

Create an environment where clients are able to be at the centre of their own care and decision making processes sensitive to the needs of the older person. Respect the dignity, wishes and beliefs of the service users, involving them in shared decision making and obtaining their consent to treatment/referrals.

Act to safeguard the patient taking emergency action where necessary, reporting any potential or actual risks to the patient immediately and following the safeguarding policy. Take responsibility for ensuring risks to safeguarding are reported in a timely manner within the team.

Provide leadership, mentoring and support for junior health and social care employee and to implement, monitor and evaluate the effectiveness of the nursing competency framework.

Work collaboratively to prevent unnecessary hospital admission and facilitate early hospital discharge.

Undertake comprehensive interdisciplinary assessments and plan, deliver and evaluate interventions and/or treatments when there are complex issues and/or serious illness.

**Provide holistic high quality care involving**: patient assessment, examination, investigation, treatment and discussion with relevant practitioners referring to appropriate service providers when/where necessary.

Work in accordance with the NMC Code of professional Conduct.

Ensure evidence based care is practiced and promoted at all times.

Act as a clinical lead for the Early Intervention Community Service advising on the care and management of the person with complex short term health care needs.

To lead and develop an equitable Early Intervention Community Service by integration and multidisciplinary Teams.

Facilitate and contribute to the development and implementation of streamlined patient care pathways across the local Health and social care economy.

Be able to perform and lead on the development of clinical skills.

To lead the implementation of national and local quality initiatives.

Alongside the Operational Manager, play a lead role in the improvement of services and development, implementation and evaluation of protocols and clinical guidelines.

Assess for, prescribe and provide non-complex equipment and devices within scope of practice.

Maintain responsibility for reviewing outcomes of treatment and care given by all members of the team for individual client caseload.

Manage own time effectively, prioritise and delegate workload activities.

Responsible for ensuring effective communication verbally and in writing with colleagues and stakeholders, utilising electronic methods of communication within the remit of the Data Protection Act ensuing clinical information is conveyed within a timeframe that ensures patient safety.

Initiate and lead multidisciplinary team meetings.

Participate/initiate, conduct, analyse and disseminate audit and evaluation findings: implementing changes to improve patient care.

Proactively work with the Early Intervention Community Service in the management of risk.
