We are looking to recruit 2 dynamic Discharge coordinators to join our site.

The post holder will have responsibility for supporting the Patient flow improvement lead with the overall flow of the site. This will include identification of suitable patients using an active pull model, liaising with the wards, attending board rounds, facilitation of safe discharges, attending and escalating any delays from weekly length of stay meetings, working closely with social workers, case managers and other outside agencies.

The post holder will also support the coordination of patients through direct conveyancing and ensure our beds are efficiently coordinated and optimised
- Escalating discharge delays
- Attending SAFER board rounds
- supporting and implementing patient choice
- Working closely with RLH/AUH to actively pull suitable patients
- Supporting Patient improvement lead to increase flow throughout the side and reduce LOS

Liverpool University Hospitals NHS Foundation Trust was created on 1 October 2019 following the merger of two adult acute Trusts, Aintree University Hospital NHS Foundation Trust and the Royal Liverpool and Broadgreen University Hospitals NHS Trust.

- Identify suitable patients for the site
- To liaise with multi-disciplinary team regarding discharge arrangements of patients on the site
- Discussing with families if patient choice issue
- To ensure completion of relevant documentation required, TTO’s and discharge letters to aid rapid discharge.
- To liaise with other agencies, services and relatives/carers as required to provide effective communication to enable timely and safe discharge / transfer of patients.
- To organise transportation required to aid discharge.
- To be responsible for discharge planning from the point of admission ensuring that discharges are expedited in a timely manner and reducing length of stay
- To work with the multi-disciplinary teams to ensure patients, relatives and health professionals both inside and outside the Trust are all working together to achieve the same goal of safe and seamless discharge.
- To be the single point of access whilst on duty for all discharges within the area of responsibility.
- To progress and chase patients through their hospital journey and ensure patients, relatives and colleagues are kept up to date with progress and delays.
- To identify problems in relation to discharge and initiate actions to resolve them in order to facilitate safe and efficient discharge.
- Throughout the shift ensure that beds are used efficiently and appropriately and the discharge lounge to support the flow of patients through the Trust.
- To liaise with the MDT team to ensure patients with complex needs have adequate community care packages in place to meet their individual needs.
- To organise and co-ordinate with patients, relatives and various outside agencies the requirements of patients with complex discharge needs.
- To report daily and escalate information relating to any delayed discharges and regularly review actions being taken to address the delays.
- Communicate with nursing and medical colleagues ensuring all staff are up to date on discharge plans and projected discharge dates for each patient.
- To effectively utilise capacity and operational knowledge as required to ensure appropriate patient flow

Knowledge, Training & Experience
- The post holder should have proven experience in working in a busy environment, being able to interpret complex information and communicating it effectively to the appropriate person.
- To have a working knowledge of how hospitals are run, what team functions exist and how they interlink.
- Develop effective working relationships built on trust and cooperation
- To understand operational management and how their own role contributes to this.
- Good IT skills to enable the access, collation and communication of data/information to the relevant staff.
- Identify problems as they arise and support decision making to provide solutions.
- Have experience in patient discharge.
