About the Role

The Patient Services Coordinator is a vital member of our clinical operations team, responsible for scheduling visits and communicating with field staff, patients, physicians, and other stakeholders. This role requires strong organizational and communication skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Serves as a backup during peak periods, including receiving calls from the field staff and assisting with weekly case conferences.
  • Maintains the client hospitalization log, including entering coordination notes and sending electronic logs to relevant staff members.
  • Schedules discharge visits, OASIS collections, and recertification visits following case conferences.
  • Coordinates TIF OASIS collection visits and deletes remaining schedules.
  • Reviews lab reports, assesses normality, and routes reports to medical records and physicians as needed.
Requirements
  • At least 1 year of home health experience.
  • Coding certification is preferred.
  • Valid state driver's license and automobile liability insurance.
