This exciting opportunity is for an activities co-ordinator to join our team to work 37.5 hours a week, which will be spread over 5 days during the afternoon (late shifts - from 11h30 to 19h30) and will include the occasional weekend work according to patient’s needs.

We have a post available on Shand Stroke Unit (Mixed Speciality Ward - Stroke and Neuro Rehab and Acute Medicine based at Bedford NHS Trust).

The post holder is required to support qualified nurses and other professionals within our multi-disciplinary team.

Ward staff will identify suitable patients for the Activities co-coordinator to work with on a daily basis; this would normally be those requiring additional support or supervision. Providing a calm approach with emotional validation and interaction can often improve the experience and interaction can often improve the experience and outcomes for patients with dementia and/or a level of neurological deficit and their carers.

The post holder will be responsible for the provision, development and implementation of activity based patient group sessions and sessions focusing on group and individual 1:1 work. The post holder will also support the multi-disciplinary team in providing a comprehensive programme of activities to meet the needs of the patient group.
- To promote person-centred care by ensuring the use of the ‘This Is Me’ biography.
- To provide cognitive stimulation through activities, social interaction and engagement to improve health and wellbeing.
- To provide instruction (verbal and non-verbal) to patients on a range of activities as part of the activities programme.
- To develop a rapport with patients to encourage and motivate them to participate in activities (either within a group or individually).
- To contribute to the development and implementation of a creative and innovative activities programme responsive to the varying needs of the patient group.
- To maintain and update an activity plan on the ward
- To observe general behaviour, abilities and responses of patients and report them verbally to the multi-disciplinary team and record within the patient record.
- To provide support at mealtimes, encouraging oral intake and nutrition.
- To continue therapy goals set by therapists with the aim of improving functional outcomes.
- To support ward staff in managing stress related behaviours

1. Person centred care will be promoted using biographical information obtained from carers and patients. This will help in the delivery of an effective care plan to optimise outcomes for the person with Dementia. The Activities co-ordinator will be expected to communicate with carers and the patient to obtain relevant information to facilitate this approach. This may include telephone contact with care home staff to complete a ‘This Is Me’ or facilitate good communication and rapport with carers or relatives visiting the person with Dementia.
2. Cognitive stimulation will be provided by the Activities Co-ordinator using resources available in the hospital and the involvement of therapy and family where appropriate. The Activities co-ordinator can help identify relevant resources required to improve the service provision. Cognitive stimulation may be provided in groups (no more than 4) or on an individual basis depending on the patient’s needs on that day.
3. Nutrition and hydration are important aspects of care and recovery for patients in our care, the activities co-ordinator will assist patients to meet their nutritional requirements and work to promote a relaxed mealtime which is conducive with social interaction. Assisting some patients to feed will often be required during mealtimes and in-between meals with supplementary fluids and nutrition. Identifying special needs and dietary preferences will help deliver a person centred approach to mealtimes.
4. Ward therapists may set goals for patients to improve functional outcomes and the activities co-ordinator will be working towards achieving some of these goals during their daily routine with patients. Examp
