The hospital at home Matron is part of the leadership team within Principle Medical Limited & is responsible for the management and development of the hospital at home service. The role is diverse and encompasses:
Delivery of activity in line with key performance indicators.

The Matron will work alongside the operations manager, & the Clinical Care Coordinators to:

**Main duties of the job**

**The post-holder will**:
Support patients who would otherwise be in hospital to receive care, monitoring, and treatment they need in their own homes.

Use expert levels of clinical decision making through assessment and interpretation of clinical information to support patients.

Identify predictors of adverse outcomes in older and frail community dwellers and tailor limited resources accordingly with services from primary and secondary care.

Provide proactive case management of patients, addressing unmet needs and improving care coordination and wellbeing.

Collaboratively work with members of the multidisciplinary team, community, and primary care in order to identify those at risk of admission and provide a coordinated responsive care.

Provide strong professional leadership to enhance professional standards of care cross the service.

Lead and participate in research and audits to support patients with complex health conditions.

Frailty Virtual Ward Team:
Contributes to the delivery if outstanding patient care by:
Supporting frail older people with complex health care and social needs.

Clinical care and coordination of services.

Identification of risks and prioritisation of care.

Timely decision making to inform the mobilisation of services.

Monitoring support of patient in their own homes.

Support the development of the service.

PML is a successful not-for-profit, GP-led organisation providing various NHS community and primary care clinical services to patients across Oxfordshire and Northamptonshire. To make clinical judgements surrounding complex health concerns or situations which require analysis, interpretation, and comparison of a range of care options to enable initiation of interventions to support patients healthcare needs.

Provide and support effective discharge planning.

Uses advanced communication skills in all aspects of patient assessment, care, and support.

Educates patients and their carers on issues surrounding health promotion and illness prevention. Promote and facilitate changes of lifestyle where appropriate.
