Job Overview

We are seeking a highly skilled and experienced Community Nurse Team Leader to join our team in Bicester. As a key member of our district nursing team, you will be responsible for providing strong clinical leadership and ensuring the delivery of expert evidence-based nursing care to patients in the community, their own home, or other care settings.

Main Responsibilities
  • Provide clinical leadership and management of the team
  • Accountable for the management, development, and supervision of identified staff within the community team
  • Overall accountability and responsibility for the named district nursing team
  • Clinical holistic assessment and treatment of complex patients
  • Work in partnership to reduce hospital admission and facilitate discharge
  • Promote optimal levels of independence and wellbeing for patients
