Children's Complex Care Nurse – Community Based
Job Summary
We are seeking an experienced Paediatric Nurse to join our team at Thornbury Community Services. As a Children's Complex Care Nurse, you will be responsible for delivering high-quality care and support to young people with complex needs in a community-based setting.
Key Responsibilities
- Provide individualized care and support to children with complex needs, including those with cerebral palsy and other neurological conditions.
- Work collaboratively with multidisciplinary teams to develop and implement care plans that promote the child's physical, emotional, and social well-being.
- Conduct thorough assessments and evaluations to identify the child's needs and develop strategies to address them.
- Administer medications and treatments as prescribed by the child's healthcare team.
- Provide education and support to the child's family and caregivers to promote their understanding of the child's needs and how to meet them.
Requirements
- Minimum of 6 months' experience in paediatric nursing, with a focus on complex care.
- Recent experience in key skills such as nasopharyngeal suction, mechanical cough assist, care and use of gastrostomy, and epilepsy management.
- Strong communication and interpersonal skills, with the ability to work effectively with children, families, and multidisciplinary teams.
- Ability to work independently and as part of a team, with a flexible approach to meet the changing needs of the child and the service.
