Job Title: Paediatric Community Nurse

We are seeking an experienced Paediatric Community Nurse to join our team at Thornbury Community Services. As a Paediatric Community Nurse, you will be responsible for delivering high-quality care and support to young people with complex needs in a community-based setting.

Key Responsibilities:
  • Provide bespoke care and support to young people with complex needs, enabling them to stabilise at home and achieve personal goals.
  • Work collaboratively with a multidisciplinary team to develop and implement individualised care plans.
  • Conduct regular assessments and reviews to ensure the ongoing needs of young people are met.
  • Develop and maintain strong relationships with young people, their families, and other healthcare professionals.
Requirements:
  • Minimum of 6 months' experience in the last 2 years in a Paediatric Community Nursing role.
  • Recent experience in key skills such as nasopharyngeal suction, mechanical cough assist, care and use of gastrostomy, and epilepsy.
  • Strong communication and interpersonal skills, with the ability to work effectively with young people, families, and healthcare professionals.
