Pathway Coordinator in Health Inequalities Project

An opportunity has arisen for a pro-active, dynamic coordinator to assist with our new Health Inequalities project, providing administrative support and tracking patients through the PTL, organising diagnostic tests and chasing results.
The post holder will assist the management of the PTL waiting list initiates to address health inequalities, thus waiting times target across the Trust.
To work with the perioperative team providing support across hospital sites and divisions.
Responsible for data collection and analysis of patients on the surgical pathway and entering this information on to the database.
Track designated patients throughout their pathway, escalating issues of concerns promptly, escalating issues of concerns promptly.
Liaise with referring clinicians and primary care to ensure all patients being transferred for treatment are optimised at the earliest opportunity.

Main
**Responsibilities**:
The post holder will work closely with health inequalities team to provide an administrative service to support the patient pre-operative pathway.
To liaise with patients and primary care providers to ensure all patients have appropriate investigations and follow up.
Record details of patients referred via other routes other than the health
inequalities dashboard. Report to the Service Management teams any deviations from process.
To administer entry into the database according to guidelines and monitoring milestones.
Undertake validations and corrections ensuring the pathway is kept up to date.
Work closely with the Trusts performance teams to contribute to regular
validation reports on patient’s pathways and ensuring accuracy of data at all times.
Ensure data is entered in a timely manner, checking accuracy and detail at all times.
Negotiate with all clinical and administrative personnel, within the Trust, tertiary and primary care centres to establish good working relationships in order to improve the coordination of the patient journey.
Participate in weekly tracking meetings with Service Management, highlighting any issues with patient pathways which may cause delay to surgery.
Maintain patient tracking logs by updating appointments, clinic outcomes, results, updates from other Trusts and steps in patient journeys on a daily basis.
To collect and input data, in line with Trust Access Policy and operational procedures on to the relevant Trust system.
To develop strong working links with relevant members of the Trust, wider stakeholders and a number of external staff from referring Trust teams and across the network.
To ensure any patient notes for investigation or audit are ‘tracked’ accordingly.
To identify any missing information that cannot be found after thorough investigation and highlight this promptly to the Directorate service management teams.
To highlight individual patient timeline data to appropriate clinical teams within the Trust.
To provide clerical support relating to the health inequality team as required.
To be involved in patient pathway investigations with the aim of identifying blockages, duplicates and subsequent areas where the patient journey can be streamlined.
To provide accurate data management on any relevant waiting lists.
To support all services and cross cover the other members of the team during periods of annual leave or absence as required.
Audit and Clinical Governance
Undertake demand and capacity studies where appropriate, as directed by the service management teams.
Report changes to the database on a weekly basis.
Ensure all data collection and recording of data is completed accurately for the allocated specialties.
Establish effective communication networks with clinicians and other members of the multidisciplinary teams and actively participate in development and change to any systems.
Actively contribute to surgical pathway reviews
Assist the service delivery teams in reporting regularly to the lead clinician/Trust on compliance/non-compliance of standards.
