We have an exciting opportunity for an enthusiastic and dynamic Patient Pathway Coordinator to join the Admissions team within the Gynaecology Service.
You will work within a busy team and be responsible for the organisation of surgical and non-surgical admissions. You will primarily be responsible for organising all aspects of an admission, as requested by the specialty teams and must be able to demonstrate excellent communication skills with patients, families, and staff.
This is a varied, busy and demanding role and we require a proactive and confident person to fill the post. Excellent communication and organizational skills are also essential, along with proven hospital administration experience.
Take responsibility for the organization of surgical and non-surgical admissions within Gynaecology Service.
Take responsibility for ensuring that theatre lists are booked in line with the trusts 6-4-2 policy, ensuring that theatre time is used optimally and escalating any issues that may arise to the Administration manager.
Take full responsibility for producing and circulating accurate admissions lists on a weekly basis.
Demonstrate excellent communication and interpersonal skills with staff and stakeholders.
Communicate with patients and their families/carers both verbally and in writing confirming preop and admission dates.
Answer and respond to any queries or concerns raised by patients and their families/carers, staff and other stakeholders related to their admissions.
Contact and liaise with support services and other specialties as needed by the patients and their families i.e., transport and interpreters bookings. Ensuring that documentation is sent to the necessary departments and parents/carers are also notified.
Ensure that all patients are seen within the 18-week referral to treatment (RTT), or 6 week wait diagnostic pathways.
Rebook all hospital-initiated cancellations within 28 days, considering the patient waiting time avoiding potential breaches and ensure all actions are communicated to the Admissions Manager and relevant Service Managers promptly.
