Berkshire Healthcare NHS Foundation Trust is currently seeking a Mental Health Practitioner (CPN120) to join their Community Mental Health Team based in Maidenhead. This role will be full time Monday to Friday 9am - 5pm for the next 12 weeks.

In this role you will be located in and working across an area with all the benefits of country living with the convenience of big city life.

**What you will be responsible for**
- Initiate and carry out the agreed care plan.
- Be able to identify and select from a range of health, social and voluntary agencies which could assist and improve the care of individuals and their families.
- Monitor and evaluate the agreed care plan, making adjustments as appropriate in collaboration with the wider team by reporting in review or at CPA meetings.
- Manage and be responsible for an identified caseload of patients.

The role will involve, but not limited to you performing the above.

**You will have the following qualification/experience**:

- A minimum of 6 months paid experience as a Mental Health Practitioner for a UK hospital environment within the last 3 years.
- Comply with NMC Professional Registration and keep up to date with current nursing practices
- Ensure accurate records are kept in accordance with CHS policy and NMC guidelines
- Undertake regular mandatory training*
- Full UK driving licence and own vehicle.

**Depending on the Trust, you may require variable additional certification and skills.**
