Patient Flow Co Ordinator

To establish and maintain robust multi-disciplinary discharge planning arrangements in order to discharge patients safely and reduce length of stay (LOS), delayed transfers of care (DTOC) and re-admissions
To proactively co-ordinate, review and case manage complex discharges and to facilitate simple discharges by engaging the multi-disciplinary ward team
To be visible at ward level and to demonstrate consistent leadership and best practice in discharge planning
To focus on improving the patient experience through seamless multiagency working
To develop effective working relationships and maintain clear lines of communication with clinical and managerial staff both internally and external to the Directorate/trust
To effectively capture any delays/cancellations in terms of diagnostics, treatment, referrals and escalate accordingly. Take responsibility for bed management across inpatient wards
Review patient data according to evidence based LOS tool and input information into electronic software
Analyse and interpret LOS data and action accordingly
Collate and report monthly delayed discharges
Ensure compliance with agreed nursing policies, procedures and protocols within the wards
Attend daily board rounds on inpatient wards with members of the Multi Disciplinary Team (MDT) to resolve issues that are preventing patients discharge
Attend weekly Multi-Disciplinary event (MDE) with wider MDT to discuss patients with extended LOS
Attend daily doctors meeting 9am/4pm to discuss beds
Arrange and Chair Multidisciplinary team meetings for individual patients with very complex discharge needs
Refer and liaise with Social Service Department and other local agencies
Complete nursing assessment of patient’s requiring placement into Nursing or Residential Home. Co-ordinate the completion of assessments from other disciplines
Perform discharge “talks” with patients to ensure they have optimal information and knowledge of their discharge arrangements and what to do if they require help or support
Liaise with outlying centres for rehabilitation/repatriation to ensure timely transfers
Co-ordinating the multi-disciplinary team to prevent readmission and address problems that may occur in failed discharge
Education for all health care professionals regarding the principles of discharge co-ordination
Work to Trust bed management escalation algorithm and action accordingly

Planning and Organisational Skills
Organize and prioritise own workload within agreed timescales building in flexibility to deal with unforeseen demands on time and resources
Be familiar with the use of Electronic Patient Records (EPR) systems and any other computerized system integral to the running of the service
Recognize the need for change and be proactive in delivering service improvements to both patients and staff
Work closely with clinicians and admission managers to ensure that length of stay is appropriate
Utilise advanced communication skills, including verbal and written when dealing with highly charged and complex situations
Communicate sensitively and empathetically, diffusing situations of conflict when tensions arise around clinical priorities
Work collaboratively with the patient flow manager to implement key divisional discharge process priorities, e.g. use of white boards, predicted discharge data
Physical Skills
During hours of working monitor that ward areas and equipment are kept clean and tidy to ensure the national cleaning standards are achieved and report any deficiencies to the ward manager
Responsibility for Patient and Client Care
Maintain currency with clinical developments, ensuring clinical understanding of these and their implications on the patient pathway
Utilise clinical expertise to advise on the movement and placement of patients, for example, according to clinical dependency and infection control requirements
Challenge the multi-disciplinary team on the progression of treatment plans, addressing areas of concern and offering clinical advice where appropriate
Assist with the development of appropriate audits in relation to bed capacity and the development of action plans in the light of findings
