Actively demonstrate excellent written and verbal communication skills with all members of the multi-discplinary team Responding to telephone enquiries and relay messages accurately, courteously and competently Welcome patients and visitors in a professional, friendly and helpful manner Work effectively as part of a team, supporting senior colleagues to implement agreed Discharge Plans Communicate effectively and sensitively with patients, relatives and colleagues, referring to senior colleagues as necessary To attend weekly Lengthened Length of Stay meetings Be present at the Daily COW MDT Board Round as required, and produce an action plan following attendanceAnalytical and Judgement skills Be responsible for maintaining an accurate and up to date bed position for the Trust, emailing the information out as required To comply with the Trust and Departmental escalation plans, ensuring that relevant senior personnel have been informed of the position and appropriate action taken On instruction, forward plan and organise with the CDU the availability of beds and inform the CDU co-ordinator when a bed becomes available Attend case conferences and Best Interest meetings as required, take minutes and produce accurate reports of treatment/action plans discussed. Effectively monitor patients progress towards discharge, taking action to ensure delays are promptly addressed Assist the Patient Flow Team Leader with audits, data collection, and review of bed utilisation, producing reports to demonstrate results (use of excel)Planning and Organisational SkillsLiase with staff to facilitate the safe and timley discharge of patientsTo chase any intervention that a patient may requireWith the support of the Patient Flow Team Leader and Co-ordinators to prioritise own workloadWork within given timescales and deadlinesBe available at the daily COW MDT Board Round to be able to capture any delays in Patient FlowAssist the Patient Flow Team prepare information for the weekly Length of Stay meetingAssist the Patient Flow Team to prepare the data collection for the monthly reporting of Delayed Transfer of Care informationResponsibility for Patient and Client CareDemonstrate effective and clear communication with patients and their families, keeping them involved in discharge planning at all timesAssist with the telephoning of patients post discharge for a follow up callResponsibility for Policy and Service DevelopmentWork towards a Standard Operating Procedure and Trust policies for Admissions, Discharge and Transfer policies.Responsibility for Information ResourcesComputer LiterateAbility to use emails safely and securelyEnter data in to computerised systems as requiredKnowledge of Excel and able to useTo ensure the security and confidentiality of patients notes and information is maintained at all times as per Trusts Information Governance PoliciesAbility to photocopy patient data and email on as appropriate under the guidance of Patient Flow TeamContribute to the inputting and interpretation of data on Length of Stay, Bed Occupancy, and Delays in care, to ensure accurate information is available to enable the Trust to monitor Patient Flow activity3. Special Working Conditions On a daily basis deal with patients and relatives of patients who are receiving treatment for cancer On a regular basis deal with patients and their families who have received bad news and are having difficulty coping with their condition All staff have a responsibility for Data Quality. This will vary according to individual jobs in line with the Trust Data Quality Policy
