Patient Flow/experience Co-ordiantor

To provide a single point of contact on a named ward/wards for patients, families, carers and associated people and co-ordinate/contribute to the safe and timely hospital discharge in partnership with other multi-disciplinary colleagues. Ensure that patient and family/carer have clear expectations regarding anticipated length of stay Maintain momentum of discharge planning throughout the entire process; supporting and working in partnership with other members of the MDT; doctors, Nurses, occupational therapists, physiotherapists and other hospital assessment and discharge team members. Establish and maintain productive integrated working relationships with social service and community healthcare colleagues as required. To develop strong working relationships with ward teams, Discharge Lounge, Therapy teams, Discharge co-ordinators pharmacy etc., to support and co-ordinate timely and appropriate assessment and safe discharge of people from acute beds.
