You will be working in partnership with other professionals and agencies to promote person-centred outcomes for individuals with care and support needs and within communities.
Experience
Demonstrate an understanding of the needs of older people, people with physical disabilities, adults with learning disabilities and sensory impairment and their carers
Evidence of interest and aptitude for work with older people, people with physical disabilities, adults with learning disabilities and sensory impairment and their carers and/or related work
Relevant experience of working with older people, people with physical disabilities, adults with learning disabilities and sensory impairment and their carers and/or related work.
Ability to work in a person-centred way, respecting and valuing all.
The post holder must possess a full driving licence
