Care and Support Practitioner

Job Description
- Provide personal care and support to customers in their own homes, including assistance with daily living tasks, medication management, and meal preparation.
- Develop positive relationships with customers, their families, and colleagues, promoting a culture of respect, dignity, and compassion.
- Work collaboratively with other healthcare professionals to ensure seamless delivery of care and support services.
- Maintain accurate records and documentation, adhering to company policies and procedures.
Requirements
- Full and valid UK driving license.
- Access to a car/vehicle.
- Right to work in the UK.
- Excellent level of English.
