Our Community Safety Practitioner (CSP) team has a vacancy for an individual with emergency department experience, excellent communication skills and experience in leadership and multi-agency working. You must have an elevated level of autonomy, interested in developing specialist skills, managing complex risk, promoting safety, improving quality, and working in partnership with third party agencies and stakeholders.

To recognise vulnerable patients that attend the Oxfordshire emergency departments identifying attendance patterns, developing, planning, and reviewing patient centred management plans for patients who present to the emergency departments with issues including alcohol, illicit substance misuse, homelessness, and violence.

The CSP's work closely alongside the high intensity user team, the alcohol care team, safeguarding teams and security lead.

We aim to provide brief intervention as well as more extensive community follow up. Using innovative and creative ways to engage patients who are at times hard to reach and chaotic.

We are a small team of committed individuals where you will be supported and guided into this bespoke role that covers both the John Radcliffe and Horton General Hospital.

Facilitate the collection, analysis and interpretation of complex data regarding patient presentations related to community safety issues

Provide reports on this information to Public Health and other statutory services, including the Community Safety Partnerships and Thames Valley Police in line with the Crime and Disorder Act 1998

Provide brief interventions relating to alcohol consumption and or drug misuse +/- referral to specialist services

Provide advice and intervention to homeless patients, or those vulnerable housed, attending ED +/- referral to specialist services as required

Lead on individualised patient management plans to reduce harm using a client-centred approach.

Write management plans in collaboration with internal departments and external organisations

Enable potentially hostile patients to recognise and agree to the need for change in their behaviour and motivate the patient to make this change

Recognise recurrent patient attendances and determine appropriate action. The post holder will manage the process of identifying patients most at risk, determining appropriate action, and negotiating agreement and best outcomes for client. This will require a high degree of autonomy and regard for working with a wide range of services and partners

Act as a resource for ED clinical staff relating to the identification of community safety issues and the appropriate signposting of patients to services
