The relocation package is for anyone who is joining CWP in a clinical role at Band 5 or above, who would need to relocate in order to take up employment with Cheshire and Wirral Partnership.

An exciting opportunity has arisen to join a well-established Community Mental Health team based in Macclesfield that serves the residents in East Cheshire.

We are looking to recruit a clinical lead, specifically with skills in Older people's mental health care, who is enthusiastic, self-motivated and who provides confident support and leaderships to clients and staff. Good communication skills, an ability to work in partnership with service users and carers and act to safeguard clients where appropriate are essential qualities.

As clinical lead, you will deputise for the Team Operations Manager, provide mentoring and support for team members and implement, monitor and evaluate the effectiveness of the clinical standards within the team.
- Providing clinical oversight, guidance and supervision of the CMHT staff to ensure quality patient care.
- Bring clinical leadership and knowledge of supporting older people with complex mental health conditions including organic (i.e. Dementia) presentations.
- Coordinating patient care, monitoring progress and ensuring effective communication between teams to ensure seamless care.
- Implementing high standards of patient care within trust guidelines and policies.
- Provide mentoring and training to enhance the teams skills and knowledge.
- Assisting the team operations manager in the collection and analysis of patient outcomes and team processes to make informed decisions and improvements.
- To advocate for the needs and rights of the patients and ensure that there concerns are addressed promptly and effectively.
- Support the team to identify and manage clinical risk.
- To effectively collaborate with other teams (both external and internal) to enhance the patient experience.
- To engage in continuous improvement initiatives to enhance patient and staff satisfaction that also maximised operational efficiency.
- To provide strong leadership fostering a positive, optimistic and productive work environment.

CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.
