Your role will support cultural change and ensuring there is effective management in place for the programme. You will be working on behalf of the council and Frimley ICB to achieve this.

You will be leading an innovative new programme to reduce health inequalities and maximise wellness through a focus on prevention. We think we can do this by supporting and encouraging increased autonomy, community action, activities, and asset development. ‘People helping people to remain well’. You will develop the strengths of health and social care organsiations to be more effective working directly with communities.

What it’s like working in our team

Thriving Communities is a new programme, with plenty of scope for you to lead the work with independence. However you will be part of a well established Chief Executive’s Office team who lead on a range of functions including community engagement and equalities, business change, policy and performance and communications and marketing.

You will be supported to carry out the following duties, responsibilities and training
- Establish a pilot project for the Thriving Communities health and prevention programme, based in a specific geographical area, later expanding to other communities based on learning and insight.
- Initiate and drive a transformation in the relationship between organisations in the health and care system and communities; founded on listening and collectively acting on the voices of people with lived experience, co-design, and co-production. This involves a cultural shift which will need to be enabled by organisational development including training and development activity.
- Work with partners, including Frimley ICB and the voluntary, community and faith sector, to manage and coordinate the delivery of a programme of activity to reduce health inequalities, maximise wellness through a focus on prevention and increasing self-care and community resilience through community development.
- To develop and be accountable for ensuring effective programme management including evaluation, outcome reporting and budget management through developing an evaluation framework and effective governance.

What we’re looking for

We are looking for a highly self-motivated, inclusive and resilient person, experienced in working in community or health and social care focused environments. You will be able to coordinate an innovative strategic programme, including the practical governance and budget management. You will need to be skilled at working creatively, collaboratively and developing effective partnerships.
