We are looking for motivated pathway lead to join our developing Integrated offender Health service, The service is delivered in partnership provided by Midlands Partnership NHS Foundation Trust (MPFT) and North Staffordshire Combined Healthcare NHS Trust (NSCHT) that brings together the following existing services into a single integrated offer:

- Liaison & Diversion - assesses vulnerable individuals with complex needs being brought into the criminal justice system, having been accused of criminal activity; including but not limited to support with mental health or a learning disability, alcohol or substance misuse, homelessness, financial needs or social difficulties.
- Mental Health Treatment Requirements - Supports the mental health and wellbeing through access to primary level psychological treatments as a sentencing option to address potential contributory factors to someone’s offending
- RECONNECT - seeks to improve the continuity of care of vulnerable people leaving prison by working with them before they leave and supporting their transition to community-based services.

The post holder is responsible for working closing with criminal justice partners, to ensure services function well within their environments, including the restrictions of these areas, managing the staff team specific to that pathway and ensure the safe and effective implementation of the model for that pathway.

The post holder has professional responsibility for the management of the service users receiving professional support by the team, and is required to ensure that working practices enable the supervision and direction of junior staff regarding the assessment and case management of service users. Additionally they will have responsibility for the professional practice of the team and supervision of junior staff in respect of this. This will require the post holder to ensure regular reviews and or supervision sessions take place and to oversee the systems for service user assessment, care and professional support.

Those who come into contact with the criminal justice system are far more likely to have unmet health and social care needs than the general population. By understanding and addressing those needs, we can reduce reoffending and help our service users on their recovery journey. The role is primarily will be based within the developing Reconnect pathway. The service will operate within the Reconnect service specification, working into 5 prisons within Staffordshire as well as accepting referrals from other Reconnect services for clients leaving prisons to reside in the Staffordshire area. Working alongside the other pathway leads to deliver a seamless community criminal justice pathway across the Staffordshire area.

Additional managerial responsibilities include the management of junior staff, working environment and practices, and will be accountable for the effective expenditure of specific budgets and resources.
