We are currently recruiting for a Support Worker to join a team in Doncaster providing support for adults who have a learning disability and/or associated physical disabilities.

If successful, you will provide a high quality care and support to meet the needs of adults within the service. Your core duties and responsibilities will include:

- To implement and support in the use of outcome based support plans, which are person-centred and help the individuals to make informed choices and decisions, enabling them to clarify and express their needs to achieve outcomes, manage risk and contribute to Service Planning.
- Responsibility to ensure that any concern about the alleged abuse of an Adult at Risk is addressed ensuring that South Yorkshire Safeguarding Adults Procedures are correctly followed and implemented.
- Active involvement within handover's, meetings and tenants reviews delivering an overview of tenant care where required by the shift Manager in a concise and professional manner.
- Support tenants with their daily living needs including personal, nutritional, social and emotional in line with their individual support plans. To support tenants with accessing new activities / opportunities and leisure interests to stimulate and encourage independence.
- To adhere to all regulatory and statutory obligations and Doncaster Council's Policies, Procedures and Guidelines.
