This is an excellent opportunity to join our Rochdale Team who provide support to unpaid adult and young person carers, with true hybrid working arrangements.

Full training will be provided and we are looking for applicants who can:

- Support carers on a 121 and in group settings
- Collate data accurately and provide an assessment of need
- Communicate effectively and sensitively
- Maintain confidentiality
- Manage a complex workload

Specific duties would include:

- Contacting newly referred Carers to induct them to the service and to establish their needs.
- Undertaking Carers’ Assessments, taking a whole family approach.
- Reviewing needs within 6 - 8 weeks of the initial assessment
- Signposting to relevant services and support
- Establishing individual support plans
- Maintaining our case management system accurately
