An exciting opportunity has arisen for a full-time post as a Discharge Coordinator on ward C53.

Ward C53 is a 28 bedded unit that facilitates the care for a diverse range of patients and due to the nature of these they will often have complex social situations to support and resolve.

You will work closely with the multi-disciplinary team to ensure safe and timely discharges from the ward.

This job will predominately be Monday to Friday 08:00am - 16:00pm with the potential requirement for occasional weekend working.

Main duties of the job

The aim of the role is to ensure that all patients are tracked through their ward stay, to ensure timely discharge including highlighting any escalations where necessary.

The post holder will have awareness of and escalate delays along a patient’s journey, working with the ward team, Integrated Discharge Team and Bronze on Call as required to minimise these. The post holder will support the Nurse in Charge and bay nurses to complete any non-clinical tasks which may expedite the patient journey, thereby releasing nursing staff to better support the ward and care for their patients.

Delays to work through will include waits for diagnostic scan, wait for clinical review of reported scan, wait for completion of TTO, and wait for discharge following TTO. Actions the post holder will implement to reduce delays will include liaising directly with the clinical staff on the ward, liaising with diagnostic specialties, booking transport, and speaking with patients and relatives.

The post holder will work proactively, pre-empting delays where possible, and helping to plan for discharge when a patient is ‘maybe-home’.

The discharge coordinator will be working alongside the large multi-disciplinary team of nurses, doctors, physiotherapists, care support workers, occupational therapists and social workers. The Discharge Coordinator would be expected to liaise with the Integrated Discharge Team following the Discharge to Assess pathways as well as working with the nursing and medical staff to ensure prompt discharges from the ward via the Discharge Lounge as much as possible. They would also be expected to help the team keep the ward environment clean and tidy and be an approachable helpful member of the ward team. Other responsibilities include answering phones, liaise with patients and their relatives and chase any delays to facilitate a safe discharge.

- Evidence of a good standard of written and spoken English e.g. previous qualifications or evidence of previous presentations.
