Job Summary

As a Discharge Coordinator, you will play a vital role in ensuring the smooth transition of patients from our care to their next destination. This involves working closely with the multi-disciplinary team to identify discharge needs and develop effective plans to meet them.

Main Responsibilities

Act as the primary point of contact for discharge information, providing timely and accurate updates to patients, relatives, and carers. Collaborate with other wards, hospitals, and Trust bed co-ordinators to coordinate the transfer of patients, ensuring seamless care continuity.

Develop and maintain effective relationships with patients, relatives, and carers, providing support and guidance throughout the discharge process. Identify patients who are ready for discharge, facilitating timely reviews and assessments.
