Discharge Coordinator

About the Role
We are seeking a highly skilled and organized Discharge Coordinator to join our team at Nottingham University Hospitals NHS Trust. As a Discharge Coordinator, you will play a crucial role in ensuring the smooth and timely discharge of patients from our wards.
Key Responsibilities
- Work closely with the multidisciplinary team to ensure effective discharge planning and communication.
- Liaise with patients, relatives, and external teams to facilitate a safe and timely discharge.
- Identify and escalate delays in the discharge process, working with the ward team and Integrated Discharge Team to minimize these.
- Support the Nurse in Charge and bay nurses to complete non-clinical tasks that expedite the patient journey.
Requirements
- At least 2 years of experience in a telephone, receptionist, or clerical role.
- Excellent written and verbal communication skills.
- Able to work using own initiative and prioritize tasks to meet deadlines.
