**Job Title: Discharge Coordinater**
**Location: Nottingham University Hospital, Queens Medical Centre**
**Hours: 37.5 hours per week - Monday to Friday 08:00-16:00**
**Trust Location: Nottingham University Hospital, Queens Medical Centre, Derby Road Nottingham NG7 2UH**
**What you’ll be responsible for**:
- This role may require you to show evidence of vocational level 3 qualification or at least 2 years relevant experience._
**You’ll learn the following whilst working at the trust**:
- An in depth understanding of the roles and responsibilities involved in working within the NHS
- Knowledge of the systems used, to effectively complete your role to the highest standard at all times
- A sense of teamwork, gained through working alongside and supporting colleagues from all levels, within the organisation
**You’ll have the following skills/experience**:
- To work with clerical and clinical staff to maximise the efficient and effective utilisation of the Trust’s beds.
- To have responsibility for co-ordination of the ward’s discharge processes.
- To ensure patients, their carers and members of the MDT are fully involved at all stages of the patient’s admission / discharge process.
- To utilise IT/information within the trust to facilitate better discharge on the ward and enable the ward to alter processes to improve discharge.
- Act as the focal point of contact for up-to-date information in relation to discharge communication, within the MDT.
- Support discharge across the Division when performing flexible discharge coordinator role as per standard operating procedure.
- Ensure that Nervecentre is kept up to date in relation to all and any relevant information relating to the patient’s discharge including medically safe status and transfer information.
- Ensure expected discharge date and time is clearly documented within the discharge plan and the medical / nursing notes.
- Proactively manage the expected date of discharge against the treatment plan and report any variances to the medical and nursing teams.
