Job Purpose The post holder will support the team with the discharge process to support the persons discharge fromthe community beds. The post holder will support the flow coordinator in the community beds along with MDT colleagues toensure that the discharge process is active, dynamic, and efficient. By doing this, the post holder will support that people only remain in the community bed for as long asthey need to and will optimise their length of stay, making sure flow is maintained An important element of this post will be to complete referrals to social care by collecting information fromthe MDT meetings and electronic patient records. The post holder will be expected to undertake a range of duties under the direct supervision of theflow coordinator and liaise everyday with wider MDT colleagues.

Dimensions The post will be providing direct health or social care to people either in hospital or within the community The role of the Discharge Support Administrator will be an integral part of the multi-disciplinary community beds team who will be responsible for assisting the flow coordinator with safe and timely discharges from the community beds. The postholder will attend MDT meetings, feed information to internal and external partners and at times liaise with patients, carers and their families. The post holder will be using System 1 to access and record information about patients. Key Responsibilities To carry out general admin duties to support discharge including reviewing notes, transferring information to relevant pro-formas andsharing information in compliance with CityCares policies and procedures To assist the team to safely plan a person;s discharge from the community bed To attend daily board round and MDT meetings & updating electronic patient records.

Responsible for data entry, text processing or storage of data compiled by others, utilising paper or computer-based data entry systems To use a range of verbal and non-verbal communication tools to communicate effectively withcolleagues, people that use the service and external partners. Providing and receiving complex, sensitive or contentious information, where persuasive, motivational, negotiating, training, empathic or re-assurance skills are required. This may be because agreement or cooperation is required or because there are barriers to understanding with stakeholders and patients. To evaluate discharge plans on a day-to-day basis and escalate delays.

Need to day-to-day judgements involving facts or situations, some of which require analysis. Be an active member of the Community beds MDT and to build relationship with external partners including Social Care. Might involve a degree of supervision of junior staff as per service needs.
