We currently have an exciting opportunity to join the Personality Disorder Inpatient Transition Team (PDITT) at Oxford Health. The PDITT is a pilot project funded until April 2025, and we are looking for a highly motivated and enthusiastic individual to fill the role of Mental Health and Wellbeing Practitioner on a fixed-term basis.

As a member of our team, you will work collaboratively with colleagues to facilitate the safe and effective discharge and transition of patients with a diagnosable personality disorder from hospital to the community. Your duties will involve closely supporting patients throughout their admission and providing ongoing assistance for up to one month post-discharge. You will also work in partnership with patients, their families, significant others, and healthcare professionals to develop and implement collaborative care, safety, and risk management plans.

Offer support to patients throughout their hospital admission. This support will involve face-to-face interaction with the patient, as well as their family and significant others.

Work in collaboration with all professionals involved in their care.

Support individuals in creating effective, personalised, crisis and safety plans.

Identify any non-clinical barriers to discharge and collaborate with the appropriate professionals to address these.

Attend ward rounds and discharge planning meetings, and actively participate in professional meetings and multi-disciplinary discussions.

Work alongside other team members to provide service-users with intensive follow-up for up to one month after they have left hospital.

The successful applicant will work collaboratively with patients, their families and significant others, alongside the patient's Keyworker and all professionals involved in their care, to support a safe transition between the hospital and community for patients diagnosable with Personality Disorder.
