Job Overview

We are seeking a highly skilled and experienced Mental Health Clinician to join our team at the Keystone Oxford MH & Wellbeing Hub. As a key member of our integrated hub team, you will play a vital role in delivering high-quality mental health services to adults living with severe mental illness.

Main Responsibilities
  • Support the Clinical Lead in developing and implementing the service and your role within it.
  • Supervise and lead a team of staff, providing guidance and support as needed.
  • Screen, triage, assess, and coordinate referrals for patients requiring mental health support.
  • Provide brief support and time-limited evidence-based interventions to patients.
  • Support patients transitioning to primary care, developing care plans, and preparing them for step-down services.
  • Map community resources and assets available for individuals with SMI within the specific PCN geography.
  • Collaborate with physical health and mental health services to provide comprehensive care.
Person Specification
  • Current professional registration: Registered Mental Health Nurse or Occupational Therapist.
  • Postgraduate education specifically relating to client group (desirable).
  • Community experience in mental health services for adults and older adults with severe/significant mental illness (essential).
  • Experience of working in primary and secondary mental health services (desirable).
