Specialist Practitioner of Transfusion

**Main Duties include**: working in close association with the Blood Scientists in the laboratory, acting as an intermediary with the nursing professions and clinicians at ward level; being part of the Trust risk management and clinical governance strategies. Developing and implementing the policies from the Hospital Transfusion Committee and responsibility for the training and auditing of these policies.

- MAINDUTIES:
Education and Teaching
1. Develop evidence-based policies, protocols and procedures from current National guidelines/recommendations, covering technical and clinical aspects of blood transfusion; ensuring that these are disseminated and adopted within the Trust.
2. Teach and assess formally/informally across a multidisciplinary spectrum.
3. Promote a development/education culture within the Trust.
4. Provides effective teaching to individual patients and their families relating to blood transfusion needs.
5. Participate, implement and develop National programmes at organisational level. (NCA, local training programmes, NBTC/BSH)
Clinical Practice
1. Promote the safe use of blood components within the constraints of clinical need and cost; in line with Patient Blood Management strategies.
2. Production of policies and protocols for the administration of blood and blood products in accordance with national guidelines and the Hospital Transfusion Committee.
3. Identify and monitor areas for nursing practice development.
4. In association with the laboratory, monitor the appropriateness of blood component usage and wastage.
5. Provide support for users of the blood transfusion service.
6. Oversee the development of autologous blood transfusion, based on a care pathway to be approved by the Hospital Transfusion Committee.
7. Develop clinical governance systems and processes to ensure provision of a high quality blood transfusion service that meets the requirements of the European blood product directive.
8. Investigate transfusion related incidents and prepare reports to the relevant bodies - HTC (Hospital Transfusion Committee) and SHOT (Serious Hazards of Transfusion)., and facilitate change processes.
9. Audit the management of patients receiving blood and blood products.
10. Work with all Trust departments to ensure that adequate advice and support are available for all patients.
11. Provide guidance and support in service planning and development.
12. Organise personal work and practice, to identify and maintain support networks.
13. Provide training updates to SLA stakeholders and staff.
ITC Systems
1. Use the laboratory information management system (LIMS) according to the authorised protocols.
2. Comply with local and national policies for the safe, secure and confidential processing, storage and retrieval of patient and other laboratory information from the LIMS.
3. Enter accurate patient details, requested tests and coded clinical details into the LIMS, requiring keyboard skills.
4. Use Trust-wide IT systems as appropriate following Trust policies, guidelines and procedures.
5. Assist in the audit of data held on the pathology laboratory information systems.
6. Use I.T. based Quality Management system (Q Pulse) for document control, record keeping and incident reporting.
7. Use computerised stock control systems where necessary.
8. Competent in the use of Microsoft Office.
- COMMUNICATION:
2. Attend and actively participate in Blood Sciences meetings.
3. Implement training programmes to meet the varying learning needs of Trust staff. Monitor training compliance at departmental and divisional levels. Audit clinical practice against blood transfusion policy and SOP’s.
4. Escalate any difficulties or problems affecting transfusion practice to the appropriate senior member of staff.
5. Work closely with the transfusion team, both laboratory and clinical based.

Management Responsibilities
1. Promote safe and effective use of blood and blood products.
2. Lead the investigation of blood transfusion related incidents and produce reports as necessary. Use Root Cause Analysis to develop and implement prevention strategies.
3. Be a key member of the Hospital Transfusion Committee and through that part of the Clinical Governance Team for the Trust.
4. Provide reports and information on blood transfusion as requested by the Trust.
5. Implement national strategies.
Leadership
1. Provide a vision for nursing and other professionals in respect of blood transfusion.
2. Chair meetings and projects as required.
3. Initiate and influence policy development in relation to blood transfusion.
