The role of the IBD Biologics and Patient Pathway Co-ordinator is integral to the Gastroenterology Department for tracking, developing and monitoring the pathways of care for IBD patients.

Working as a member of Inflammatory Bowel Disease (IBD) Specialist Team the post holder will provide a combination of support and administration duties in the delivery of services for patients with Inflammatory Bowel Disease making a direct and positive contribution to the work of the team. The post holder must exercise initiative and judgement when dealing with patient enquires, requests, referrals and results, and be able to prioritise importance accordingly.

The post holder will have responsibility for the organisation and delivery of the weekly multi-disciplinary team meetings and follow up decisions taken. Ensuring all communication fully completed and shared where required.

Main

**Responsibilities**:

- To deal with all enquiries in a responsive and professional manner to ensure a positive image at all times. To liaise with all disciplines of staff, both within and outside the Trust to ensure that effective communications are maintained.
- Complete patient care record for any non-clinical queries dealt with.
- Use judgement and initiative to prioritise the importance/urgency of patient messages, enquires, requests and results
- Act as a point of contact for the IBD Specialist Team for primary and secondary care health professionals in a responsive and professional manner whilst communicating simple and complex information relating to patient treatment, prescriptions and care requirements
- To be responsible for incoming post, assessing priority and dealing with matters as appropriate, including initiating and replying to correspondence. To follow up a wide range of issues arising from correspondence, meetings and other delegated tasks. Often negotiation and persuasion skills are required.
- Work with the clinical, management and divisional leads in identifying problem areas within individual pathways, escalating concerns in a timely and responsible fashion.
- Collaborate with all members of the multi-professional team to ensure efficient and effective services across primary and secondary care and internal and external agencies
- Liaise with various clinical areas & teams as necessary for the coordination of patient care, services and treatments including ensuring prescriptions are completed by the specialist nurses, to ensure smooth patient experience including arranging patient appointments and day case procedures where required
- Coordination of patient prescriptions; including for homecare and healthcare services
- Develop a good working knowledge of anatomy, types of treatment, the drugs and medication used for treatment, medical coding.
- Assist in the monitoring of blood results for monitoring various treatment therapies used in IBD, in collaboration with and/or supervision of the nurse specialist team.
- Recognise the importance of blood results to ensure patients ongoing treatment and management is maintained and safe highlighting any issues to the nurses when noticed
- Be responsible for data input into the national IBD Biologics registry and the national IBD registry/ DAWN system.

Assist in the development of and maintain databases and data collection for education, audit and surveillance purposes where appropriate
- To organise meetings as requested by the team including arranging venues, agendas, training materials or education sheets are prepared in advance.
- To organise the yearly transition evening for patients transferring from the children to adult care. This includes arranging venues and sending out invite letters with information about the event.
