**Patient Coordinator Role Profile**

The Patient Coordinator plays a crucial role in ensuring the smooth and efficient coordination of patient care within a healthcare service. They serve as a liaison between patients, healthcare providers and administrative staff, facilitating effective communication and providing support throughout the patient journey. The Patient Coordinator is responsible for managing patient appointments, maintaining accurate records, and addressing patient inquiries and concerns. They contribute to delivering excellent patient experiences by ensuring timely and coordinated care delivery.

Key Responsibilities

1. Appointment Scheduling:

- Managing patient appointment schedules using electronic systems or scheduling software.
- Coordinate with healthcare providers to schedule and reschedule patient appointments as necessary.
- Ensure appropriate allocation of resources to optimise patient flow.

2. Patient Communication and Education:

- Serve as the primary point of contact for patients, addressing inquiries, and providing information about services, treatment, and general inquiries.
- Educate patients on pre-appointment instructions, including necessary preparations and any required documentation.
- Assist patients in understanding and completing required forms, such as consent and other required medical documentation.

3. Patient Records Management:

- Accurately maintain and update patient records, ensuring confidentiality and compliance with relevant privacy regulations.
- Enter patient data into electronic medical records (ENR) or other healthcare information systems.
- Retrieve and organise patient medical records as needed for healthcare providers and administrative purposes.

4. Patient Support and Advocacy:

- Address patient concerns, complaints, and feedback, ensuring timely resolution or escalation as needed.
- Provide emotional support and empathy to patients and their families during stressful situations.
- Advocate for patients needs and rights, collaborating with other healthcare professionals to ensure optimal care outcomes.

5. Collaboration and Teamwork:

- Collaborate with multidisciplinary teams, including physicians, nurses and administrative staff, to coordinate patient care plans effectively.
- Participate in team meetings, contributing insights and suggestions for process improvement and enhanced patient care.
- Foster a positive and collaborative work environment, promoting effective communication and teamwork.

Qualifications and Skills:

- High school diploma or equivalent, additional certification or training in healthcare administration is desirable.
- Previous experience in a healthcare setting preferable in a patient coordination or similar role.
- Excellent communication and interpersonal skills, with the ability to interact effectively with patients, healthcare professionals and colleagues.
- Strong organisational and multitasking abilities, with attention to detail and accuracy.
- Ability to maintain confidentiality and handle sensitive information appropriately.
- Problem solving and conflict resolution skills to address patient concerns effectively.
- Adaptability and flexibility to handle changing priorities and fast paced environments.

**Salary**: £19,650.80-£25,000.00 per year

**Education**:

- GCSE or equivalent (preferred)
