The purpose of this post is to identify and facilitate the early discharge and leave for patients who will be supported in the community by the Home Treatment Team.The post holder will be productive in ascertaining an accurate borough position regarding the overall bed management status which will then be utilised by the patient flow hub for managing beds.The post holder will assist the HTT Senior Mental Health Practitioners to ensure delayed discharges are progressed through the proper channels and resolutions are met in a timely manner. They will be responsible for providing a fully comprehensive and updated gatekeeping service report to the HTT.
