To support the Directorate Manager in developing the operational direction of the discharge to assess pathway services.

Actively support the Organisations Values and beliefs and share responsibility for the provision of the Pathway model services.

To operationally manage the pathway, working alongside other key members to ensure that all members are governed within their professional boundaries.

Work collaboratively with colleagues and managers to develop partnerships within the integration programme for Stockport ensuring a co-ordinated approach in the delivery of the Services.

To work collaboratively with multi agencies and ensure that all pathways adopt the health/social care cohesive partnership where ever possible and work in collaboration with the Clinical leads in relation to the quality agenda.

Offer management and leadership within the pathway model and ensure that clinical and operational performance is optimised.

Ensure that all services are managed within their individual financial envelope.

Ensure all performance KPIs are maintained and optimised.

Operational Lead for the pathway services will ensure the efficient and effective running of the services.

Supervise the provision and delivery of the pathway services by working in collaboration with Clinical Leads and team members

Develop and implement operational management systems and process pathways specific for the individual services patient, in collaboration with services within and external to the Organisation.

Ensure that all care provided by the team members is evaluated and reviewed and efficiency maximised.

Lead with key stakeholders on the identification, development and evaluation of operational objectives and create the appropriate SOP’s, protocols and strategies to maintain efficiency and effectiveness.

Responsible for the delivery of the individual services KPI metrics.

Responsible for safe staffing and performance. Ensure the services deliver against the commissioning intention and Trust targets.

Work alongside the Clinical Leads offering critical challenge within current practice, thus ensuring that all care delivered is evidence based, effective and efficient

Ensure that all care provided by the team members is evaluated and reviewed and efficiency maximised.

Adopt a LEAN methodology and philosophy.

Employ excellent communication and highly developed interpersonal skills to enable co-ordinated team work in the delivery of efficient and effective services, representing and leading the relevant services.

Accountable for the staffing management of the services to ensure that each service is able to operate with safe staffing levels.

Lead, instigates and coordinates service redesign proposals in collaboration with the Directorate Manager.

Play a lead role in service benchmarking.

Agree and implement service audit programmes in line with Trust plans.

Ensure operational plans are in place for workforce issues e.g. vacancy/sickness absence/maternity leave.

If successful this vacancy will require an enhanced DBS check at a cost of £38. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
