We are seeking an enthusiastic individual who has a passion for improving the patient experience and supporting the patient's journey from admission to discharge.

The Patient Flow Coordinator is a key part of aiding general patient flow through the hospital by ensuring that patients are in the right place at the right time

You will be working under the direction of the Clinical Site Co-ordinators (CSC) to help facilitate the safe timely flow of patients throughout the trust working along side different specialties within the trust.

We are looking for someone with good oral communication skills who will be able to establish and maintain relationships

This is a challenging and demanding role with the individual expected to meet the ever changing demands of the trust.

To successful applicant will assist the CSC with bed allocation for all specialties by ensuring that patients are placed into the most appropriate speciality bed that reflects their need promptly and in doing so support the Trust to meet national targets.

To ensure the full utilisation of beds across the Trust by facilitating the timely transfer of patients liaising with medical and nursing staff.

To support the system of pull within the Trust, engaging with all areas that impact patient flow across as required.

To have an ongoing awareness of patient flow issues e.g. bed availability, current bed state, outliers and bed closures.

To ensure the communication of the bed state occurs on a regular basis throughout the day.

To work with the Clinical Site Co-ordinators to develop expertise and effective high quality patient flow systems.

To support the repatriation of patients from other Trusts and abroad to the Trust in line with protocol.

To follow the system of escalation regarding bed status and ensure all relevant personnel are aware.

To continue effective communications regarding all aspects of patient flow with the CSC.

To liaise with external agencies e.g. other Trusts regarding admission and transfer procedures.

To attend the daily capacity flow meetings and provide an overview of the current situation re: patient flow/capacity.

The ability to work within a high pressure environment and be able to multi-task effectively

If successful this vacancy will require an enhanced DBS check at a cost of £38 This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
