As the Discharge Support Facilitator, you will be joining an experienced and compassionate workforce. You will be working in the ward environment alongside the supportive wider hospital team to ensure that effective admission and discharge processes are undertaken with the oversight of clinical support. You will highlight and escalate any barriers to a patients discharge from hospital to the senior team and help to organise and co-ordinate discharge planning meetings.

Effective communication is key as well as being proactive with day-to-day admin tasks for the smooth and timely manner of a discharge of a patient with consideration of their representatives. We offer in house training including those that would like to advance their skills. It is essential that you are a driver due to visiting Dorset HealthCare sites both East and West of Dorset, however specific site cover is negotiable.

The Discharge Support Facilitator role is a 12 month fixed term post or secondment, Monday to Friday 09:00 - 17:00. However, could be flexible with start time, finish time and part time hours would be considered (minimum of 30 hours).

The individual will need to ensure careful discharge planning is considered from point of admission and all relevant and necessary processes are carried out that will facilitate discharge. All appropriate literature will need to be disseminated and understood by the person / carer or identified point of contact. Role will entail recording those patients who are Clinically Ready for Discharge considering the eligibility criteria and recording on inpatient clinical records.

To provide a single point of communication with Patients/Carers/Identified person to Service Partners/LA ensuring that the discharge processes are patient centred and consider the needs and wishes of the patient and those involved within their care.

To utilise IT/information within the Trust and partner agencies to facilitate improved patient flow enabling a better patient journey.

To ensure that discharge processes are followed and provide guidance when there is lack of clarity around discharge planning.
