**Job overview**:
Ward 26 at Heartlands Hospital is looking for a 12 month fixed term Band 4 Flow Coordinator to join their dynamic team. Ward 26 is a fast and exciting respiratory ward, with complex patients and discharges, specialising in Cystic Fibrosis.

The post involves the early preparation, facilitation and execution of complex respiratory discharges, in order to promote early flow and bed capacity on the ward to support the prompt admission process required by this speciality of patients.

The role will involve liaising with the MDT and external agencies to ensure the safe discharge of patients to their determined destination. The ability to develop working relationships with the relevant General Managers and Operational Managers to ensure escalation of non-compliant pathways occur and actions are put in place to address any delays.

**Main duties, tasks & skills required**:
Monitor the transport process and identify any delays or issues that the clinical team are experiencing.

Ensure that any diagnostics and investigations that are required of the patient pathway have been requested, including those at other Trusts.

Co-ordinate appointments/procedures at other departments and organisations, as required.

Be the first point of contact, dealing proactively with all enquiries, welcome and orientate patients and their relatives/friends.

Work as part of a dedicated team with staff at all levels, communicating effectively.

Deal with telephone enquiries ensuring, where possible that the caller's needs are met or transferred appropriately.

Knowledge of daily bed capacity issues via site capacity team.

Attendance at MDT huddles.

Ensure staff are aware of any information that will assist in any safe transfer/discharge.

Liaise with all members of the multi-disciplinary and Site team.

Promote all transfers/discharges within agreed timeframes.

Identify bottlenecks which prevent smooth and timely transfer of patients.

To co-ordinate the transfer of patients to allow sufficient capacity and safe placements.

Liaise with staff in Respiratory Wards, ED, CDU, FAU, Acute Assessment, Frailty Ambulatory Care Unit and Medical Short Stay for appropriate transfer of patients.

Work with the Multi-disciplinary team, including Consultants to identify problems and ensure optimum use of beds

Have regards for Health and Safety Policies and Procedures.
