**Senior Practitioner**
**Location**:Fairways House, Slackbuie Avenue, Fairways Business Park, Inverness IV2 6AA**
**Hours: 35 per week **(Service runs across 7 days, typically working one in four weekends)**
**Salary: £24,244 per annum based on 35 hour week**
**Contract type: Permanent**
**Full UK driving licence and use of own vehicle is required (business mileage will be paid)**
**_ Ready to make a real impact within your community? _**_Join us as a _**_Senior Practitioner_**_, where you'll coordinate care transitions, mentor staff, and uphold the highest standards of service delivery._
You will receive patient referrals from hospitals, GP surgeries and other health professionals within the community. Working closely with the Service Manager, you will undertake delegated duties such as direct care provision and staff mentorship. You will ensure optimal staffing levels and adapt work plans to meet evolving user needs. Additionally, you will assist in overseeing service quality through practice observations, user feedback, and support plan audits.
**What will a typical day in the life of a Senior Practitioner involve?**
- Maximising independence through coordinating transitions from hospital to home and conducting ongoing care assessments.
- Providing compassionate care in accordance with Care Inspectorate regulations, making a real difference in service users' lives.
- Collaborating with the team to maintain optimal staffing levels and adapt work plans to evolving needs.
- Empowering individuals by connecting them to additional resources and services, while also mentoring and supporting fellow team members.
**What does it take to be a successful Senior Practitioner?**
- An SVQ Level 3 in Health and Social Care (or equivalent) (preferred).
- PDA in Supervision (preferred).
- Excellent organisational skills with an ability for prioritising your workload against competing deadlines.
- Ability to accept responsibility for the overall running of the service in the absence of a senior manager.
