We are seeking an experienced Advanced Clinical Nurse Specialist/Advanced Nursing Practitioner to join our team at Highland Hospice. As a key member of our Senior Nursing and Medical team, you will be responsible for delivering high-level clinical and psychological care to patients with life-limiting illnesses and their families.
Key Responsibilities- Provide practical, emotional, and social support to patients and their families
- Work collaboratively with the Medical and Senior Nursing team to deliver care within our 12-bedded IPU and virtually to our community and acute services
- Develop and lead relationships with the Hospice MDT
- Provide on-call provision as part of the medical rota
- Excellent communication and clinical skills
- Ability to work autonomously and make complex clinical decisions
- Professional attitude and responsiveness to changing patient needs
