An exciting opportunity has arisen within the Hospital Intermediate Care Services (ICS) Team at Walsall Manor Hospital. The post holder will be part of a multi agency team supporting the hospital to achieve safe, timely and effective discharges from ward areas.

The ICS team is multi-disciplinary and across agencies. Health and Social care staff work together to support patients to be discharged from hospital into community settings on pathways. Service users are followed up in the community.

The post holder will be an integral part of the communication between the wards and other members of the Discharge Team to move discharges forward. They will be responsible for managing any issues that are causing delays in patient’s discharges from the trust and problem solving and escalating as appropriate to ensure timely resolution of any blockages to facilitating safe and timely discharges. The post holder will be expected to complete profiles and other paper work to allow patients to return home or to other bed based services.

The post holders will provide support and guidance regarding discharge pathways to members of the Emergency Department and Acute Medical Unit to assist in avoiding of non clinical admissions.

The post holders will be expected to work flexibly to meet the demands of the service on various wards in the hospital, supporting colleagues throughout the trust by problem solving and identifying appropriate pathways for patient’s leaving the trust.

Post holders should have previous experience in a hospital or social care environment and an understanding of social care pathways, they must be able to effectively communicate with internal and external colleagues of all levels.

The position will be five day working over a seven day week, you will be included in a weekend working rota where you will be expected to work one in six / seven weekends and also be included in a bank holiday rota.

Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 286,700, providing inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor houses the full range of district general hospital services under one roof. The £170 million development was completed in 2010 and the continued upgrading of existing areas ensures the Trust now has a state-of-the-art Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit facilities. Work is also well underway on its multi-million pound new Urgent and Emergency Care Centre which will house a new Emergency Department (including Children’s Emergency Department), co-located Paediatric Assessment Unit, Acute Medical Unit and Urgent Care Centre. This is scheduled for completion this year.

To work as part of the Intermediate Care Services (ICS) Discharge Team contributing towards
effective, safe, timely discharges of patients from the acute setting
- To manage a complex case load of patients with responsibility for their

discharge planning from admission to ensure discharges are expedited in a
timely and safe manner.
- To provide leadership in the promotion of rehabilitation, patient independence

and therefore earlier discharge from hospital
- Effectively monitor patient progress towards EDD, taking action to ensure

delays are promptly addressed by the relevant ward or department.
- Contribute to the effective running of the ward by concentrating on tasks,

which directly support and assist the safe and timely discharge of the patients.
This will include completing discharge planning documentation,
communicating with external agencies such as social services, patient’s
families and carers, arranging
- Work closely with members of all areas across the hospital including being the point of contact for the Emergency Department and the Acute Medical Unit to lead facilitating discharges of non FES patients from these areas that have care needs or from nursing residential homes to avoid admissions
- To contribute to the local resolution, investigation of any informal or formal complaints and carry out follow up action for those that specifically relate to discharge planning
